Who We Are
Care That Leaves No One Behind
Cadance Disease Management was built on a simple belief: the most vulnerable members of any population deserve the most attentive care. We exist to deliver that care, systematically and without exception.
Our Mission
Closing the Gaps That Cost Vulnerable People the Most
In healthcare, the people with the greatest needs often receive the least coordinated care. Newborns leaving a NICU without a clear follow-up plan. Pregnant mothers with no clinical contact between prenatal visits. Medicare beneficiaries discharged from hospital with no one to reconcile their medications.
Cadance Disease Management exists to fill those gaps. Through structured clinical programs, trained case managers, and relentless follow-through, we provide the layer of support that transforms health outcomes for people who cannot afford for things to go wrong.
We partner with health plans and providers who share our belief that prevention, coordination, and advocacy are not optional extras. They are the foundation of good care.
“The most vulnerable members of any population deserve the most attentive care.”
Our Values
What Guides Everything We Do
Compassion
Every person we serve is navigating a difficult chapter. We lead with empathy, meet members without judgment, and remember that behind every case number is a person, a family, and a life that matters.
Advocacy
Our care managers are fierce advocates. We navigate complex systems on behalf of our members, push back against barriers, and make sure vulnerable populations are not left behind by the healthcare systems meant to help them.
Coordination
Fragmented care is one of the leading causes of preventable harm. We exist to build bridges between providers, systems, and communities so that nothing falls through the cracks for the people who can least afford it.
Outcomes
Compassion without accountability is not enough. We are committed to measuring what we do, learning from the data, and continuously improving the programs that serve our most complex and vulnerable members.
